Good Faith Exam
For applicable treatments, a $39 Good Faith Exam (telehealth medical evaluation) is required before treatment.
After booking, you will receive an email with a link to complete your Good Faith Exam within 24 hours. The exam must be completed before your scheduled treatment.
The Good Faith Exam is valid for up to 3 treatments within 12 months, subject to provider assessment and continued clinical eligibility.
Please note: Completing the Good Faith Exam does not guarantee approval for a specific treatment. Your provider will determine whether the requested treatment is appropriate for you.
Mobile Travel Fee
Nexius Health provides convenient mobile treatments at your location. A travel fee is added separately based on the distance from our service area.
Travel Fee Schedule:
- 0–10 miles: $25
- 11–20 miles: $40
- 21–30 miles: $60
- 31–40 miles: $80
- 41–50 miles: $100
- 51+ miles: Contact us for pricing
Travel fees are separate from your treatment price and Good Faith Exam.
GLP-1 Weight Management
Our GLP-1 programs are provided under provider supervision and are available only to patients who meet clinical eligibility requirements.
Injectable GLP-1 Programs
Semaglutide and Tirzepatide are provided as:
4-week regimen • 1 injection per week
- Semaglutide: $349/month
- Tirzepatide: $449/month
Oral GLP-1 Program
Oral Semaglutide is provided as:
4-week regimen • 1 dose daily
- Oral Semaglutide: $299/month
Monthly GLP-1 prices are for the 4-week treatment regimen and do not include applicable mobile travel fees.
Medication, dose, frequency, and continued treatment are determined by the provider based on your medical evaluation and clinical needs.
Forms & Consents
All required patient forms must be completed before your treatment, including:
- Patient intake forms
- Medical history
- Treatment-specific forms
- Informed consent
Incomplete forms or consents may delay or prevent treatment.
Treatment Approval
All treatments are subject to provider review and clinical eligibility.
Booking a treatment does not guarantee that a particular medication, IV treatment, dosage, or therapy will be provided. Your provider may modify or decline treatment when medically appropriate.
Pricing Reminder
All treatment prices shown on our website are base prices. Good Faith Exam fees and mobile travel fees are additional when applicable.
